ASA (Additional Signature Authority) – this form is to add and delete signatories on the Parish/House accounts.

  1. Download the form from the Website
  2. Complete Account name: RC Bishop of Auckland – (parish) and account number details.
  3. Complete details for new signatory/signatories and have them sign in the specimen signature box.
  4. Attach a photocopy of either their driver’s license or passport (some form of photo identification).
  5. If you are deleting a signatory then put the persons name in “replaces signature of” space.
  6. Post the form to the Administration Department, attn: Leanne Killgour for Kerry Coleman to authorise.



AOA (Account Operating Authority) – this form is to change the number of signatures required to sign on Parish/House accounts.


  1. Complete account name and number details.
  2. Complete signing instructions i.e. 1 or 2 to sign.
  3. Complete specific signing instructions i.e. Parish Priest and one other, or any two signatures.
  4. Post the form to the Administration Department, attn: Leanne Killgour for Kerry Coleman to authorise.
